Getting short cuts and items lost back on start menu and programs menu how?

WTF? Some how my quick hand managed to misplace or mis-arrange the start menu and programs menu from the way I use to have it originally setup. The programs are still installed but the programs that we're once on my start menu are not there now. I can see them in a programs folder but not in programs on the start menu. How do I fix this problem? Picture attachment....

Update: I noticed that every time I reboot whenever I get to the desktop I see a programs file folder on my desktop. Or if I download an app and go to install it I will get a program folder made on the desktop automatically. How do I fix this problem now?

Sorry, I dunno how to fix your problems, it sounds quite annoying though. So even if you delete the folder, when you reboot or install something it comes back?!

I've fixed everything else. I just need to know how to stop getting a folder on my desktop called programs that keeps on showing up on every boot up even after I've deleted it.
