That sounds like an interesting document... :)
As far as I know, Banshee was Banshee during developement as well... it kept its codename when it went public! Not sure about this... but one heard of it as Banshee as far back as the launch of Voodoo2... it never changed names like Napalm, (VSA-100), or Avenger, (Voodoo3) did...
I did hear things about the 3Dfx Hydra and 3Dfx Medusa back in the days as well.. but I don't know if they ever were for real...
Savage3D was GX3 and ViRGE was GX2.. I think Savage4 was GX3c or something like that... not sure about the c, but it was some form of GX3 since Savage2k was GX4...

Don't know about the 'c' there either, maybe you are mistaken with Radeon256 GPU, which was Rage6C before it was turned into R100...
Found some other nice info too: did anyone know that R200 was also called 'Chaplin' and that RV250 was also called 'Sidewinder'?

The "c" is actually kind of a guess.... I know that S3 put c on some other cores when they improved them.. like the fully functional Savage2000 that never saw the light of day was the GX4c....
