Can i enable it in Quake 4 and have Triple buffering on and it has no impact in fps what so ever?

Theoretically.

I have Vsync and TB enabled and I'm not dropping to half once under 60 - so it seems to be working fine.

Triple Buffering uses more video memory, so if your system is starved for video ram, it may be a bad idea.

True -
But without Triple Buffering and vsync enabled, when the frames fall under 60 - you will get 1/2 instantly. Hypothetical: Cruising around at 60 fps, gun fight - drop to 50fps but because TB isn't enabled you now get 25 fps. I'd rather enable it and deal with not enough video memory than 1/2 the fps. Just me :)

So if i have a FX 5700 Ultra its better to leave V Sync off?

If the tearing isn't bothering you, yes. If the tearing is bad, enable vsync and triple buffering.
