I wasted 14 hours trying to get 4191 working under Mandrake 9.1. I finally found the solution- don't use 4191, instead use 3123.
Interestingly I had been using 4191 just fine under Mandrake 9.0
Hope this helps someone else... also hope that NVidia sees this message, eventually.

I compiled and installed the NVidia-supplied drivers (src rpm's) the exact same way as I did with 9.0.... installs with no issues. But when I start X, I instantly get a blank screen and the machine half-hangs (can't reboot, but does respond to alt-ctl-sysr). There is very little in any log files that helps at all.... just some AGP messages, which are not much different than in 9.0. At the end of all the X logs, it ends with: "(II) NVIDIA(0): AGP 8X successfully initialized". Under 9.0 there is much more in the X log after that point, even though it nevers uses that verbage. I used the XF86Config-4 file I made from 9.0, even commenting out feature after feature, retrying and retrying. Updated my BIOS, disabled other MB hardware, even reinstalled 9.1. Nothing. I tried everything I could think of, and searched the net high and low. My hardware: Soyo KT400 Ultra, Athlon 2100 , 512MB DDR, Adaptec 2940, PNY Nvidia Geforce 4 Ti4200. NVidia drivers: 1.0-4191
