There are known issues with the DVI interface and the newest driver (perhaps 4191 as well, but I don't think so). nVidia is looking at it and seeing what to do.
In the meantime, can you use an analog converter anywhere along the path to the screen? If it has a VGA connector, try using the DVI->VGA converter.
