there is log written by your nvidia-bug-report.sh but I think this wont help because there was an error while envoking the script:
could not open the device file /dev/nvidia0 (Input/output error)
because your blob has failed to load (see dmesg in main post)

It was a working day today. Have anyone checked this issue? I think importance of this issue is much bigger than blocker: over 9000 modern computers all over the world cannot load nvidia blob under uefi. Nvidia bug tracker: 0 replies. :bash: Give me please a resolution or estimate of issue: cannot reproduce, wont fix or in progress?

Not really, as a long time support person myself you learn to add people to your ignore user list. This way they can act like a kid any way they want and the person they are trying to get attention from is none the wiser nor cares.

I tried to load iommu=pt. kernel sayed just: "okay. IOMMU Initialized for Passthrough Mode" and than nothing about iommu but nvidia module throwed the same exception.
I disabled IOMMU in kernel and in bios - suddenly system system failed to recognize my keyboard and mouse (usb 3-4: device descriptor read/8, error -110).
I will try to make some experiments with iommu and nvidia module tomorrow than will write results here.
If anyone has expereince with this problem - please share it

Please see http://www.nvnews.net/vbulletin/showthread.php?t=174006. The NVIDIA Linux graphics driver does not currently support native UEFI configurations.

:confused: all new hardware now supports uefi. I've googled that your driver for windows have such support. Please implement this for GNU/Linux or open source for community (help)

I've finally found a workaround to this bug!

iommu in bios ON + 64M
iommu in kernel ON
add this options to grub.cfg:
vmalloc=256M iommu=pt
