I have some good news. I got it working.

I included acpi & ibm_acpi support in the kernel,
and installed the gentoo package x11-drivers/nvidia-drivers-100.14.09

Following Chapter 8 of "NVIDIA Accelerated Linux Graphics Driver README and Installation Guide", several kernel options were suggested.
This is a table of tests and results: (I can't make it look nice in this wiki)

pci=noacpi...doesn't work...Fail:edge-trigered error.
pci=biosirq...works.............Fail:module not receiving interrupts error.

So the trick on my thinkpad t61, with Nvidia Quadro NVS 140M, and Atheros wifi card, is the kernel option irqpoll.
