I think I found the root cause of the problem: it is the motherboard (on-board video chipset)!

When I pull the card out of my linux box and tested in my XP, everything seem to be working fine. So, I assumed that the problem was due to the video card/driver/configuration. After countless hours of fiddling with the card settings, yesterday afternoon I gave up and bought myself AverKey iMicro (external VGA to TV) unit. To my surprise, I saw the same behavior with the external converter as well. At that point, I realized that the problem is with the motherboard!

Now, I plan to get myself a new motherboard...
