After working with HP, here is the solution:

The xw9400 product, as far as the CPU bus, is a bit unique in the way it handles multiple processor support as memory and graphics are actually dedicated on a per CPU basis and not shared as would normally be expected. HP recommended identical video cards.

The final solution was to disable the 'ACPI Bus Segmentation' setting in the BIOS, under ‘Advanced’ -> ‘Chipset/Memory’.

This setting is there to support some legacy operating systems. Some of the older operating systems did not handle the memory controller on the Xw9300 and 9400 very well since the memory controller on these units was not on the north bridge.

After disabling this setting, I was able to run the nvidia-xconfig utility and configure 3 monitors without problems.
