As a temporary fix until you are able to get tv-out working, you should try booting without the tv cable plugged into the card. If you have not yet verified that the driver works without tv-out, then I suggest you do.
Problems with tv-out causing this kind of corruption on screen, especially the terminals, is pretty common. Your setup appears valid to me, but I don't use tv-out with my card, so I'm not that familiar with the setup.
