You or XFree is not using the nvidia driver. From your log:

Loading /usr/X11R6/lib/modules/drivers/nv_drv.o

It tried the XFree nv driver to load. Post your config or edit it to the nvidia driver. Maybe there are more entries.

...and delete the all the trash modelines
