Just completely get rid of the IHS and just make sure the HSF makes a good contact with the core/CPU die and off you go. I wouldn't feel safe with big HSF with the bare core exposed, though. I wouldn't even try to get to the core/CPU die, just lap the IHS to a perfectly flat surface and do the same to the HSF and that should be good enough.
