Default X won't start with 9400 IGP and 180.29

I can't get X to start on my Gigabyte GA-E7AUM-DS2H motherboard with integrated GF 9400. The error on the console is

NVIDIA: could not open the device file /dev/nvidia0 (Input/output error).
(EE) NVIDIA(0): Failed to initialize the NVIDIA graphics device PCI:2:0:0.

and the dmesg error is

NVRM: RmInitAdapter failed! (0x31:0xffffffff:1018)
NVRM: rm_init_adapter(0) failed

I running mostly Debian etch, but have upgraded all X related packages and the kernel to lenny. The X version is "X.Org X Server 1.4.2" and the kernel is linux-image-2.6.26-1-686 2.6.26-13.

I used the nvidia installer to install driver version 180.29, and have confirmed that the kernel module of that version number is loading. I followed the instructions and removed all nvidia-related Debian packages beforehand.

I'm using an xorg.conf file generated by nvidia-xconfig.

I've also tried compiling the Debian nvidia-kernel-source package, version 180.22, from experimental, with the same results.

One time the card worked. This was right after a BIOS upgrade (from F1 to F2) which reset all settings. I then rebooted and changed some BIOS settings and the card stopped working. I then reset all settings to the defaults but still no luck. I've also tried disabling many other onboard devices.

I have tried many different kernel boot parameters, with no effect.

The card works using the vesa driver.

Any suggestions would be very welcome!

I've now reflashed the BIOS, and each time I reflash, the card is detected properly for the first boot, but is not detected for each subsequent boot, even if I make no BIOS changes at all. The POST screen for the first boot looks a little different, and also lists the CPU as 2.66GHz instead of 2.80GHz, but Linux later correctly detects the CPU.

I have attached nvidia-bug-report.bad.log and .good.log so you can compare them.

ehci and ohci are reversed (except on the first line), and nvidia appears only in the good output. There are some similar difference in the USB part of the lspci output.

The dmesg output is harder to compare as the timings don't match, so I stripped out the timings and have attached a diff of the resulting files.

Does this new evidence suggest any easy workarounds?

I've just tried blacklisting ohci_hcd and ehci_hcd, so they aren't loaded at all.

Now lspci -v shows the nvidia card having IRQ 20, but /proc/interrupts still doesn't list it at all, and shows nothing else using IRQ 20. And the card is still not detected properly.

I updated grub from etch to lenny and ran

grub-install "(hd0)"

and after a reboot, X works fine.

I have the same motherboard (Gigabyte GA-E7AUM-DS2H) and getting same errors.

Fatal server error:
no screens found

I upgraded my BIOS (from F1 to F3A) to the latest version, but no luck.

As jdchristensen suggested, i have tried grub-install, but nothing has changed.

Any other suggestions?
