Just a theory, but maybe because the yields on the chip have been low, they're having to perform extra quality control, hence a low failure rate for chips that do make it out of the foundry.

So what companies are we talking hear? Were they both Galaxy? I think it is all in what company you go with. I agree that 12 to 20 hours in a stress test you are just asking for a melt down!!

So let me get this straight. Every single card is made by Nvidia and then purchased by individual companies who slap their sticker on it????

Technically, NVIDIA doesn't manufacture the cards. They design the chips and such, and then they're built by an outside firm, who then slaps the stickers on.

Yeah, the don't even make the chips, the just give the design to TSMC who makes them. Then the chips go to the board companies, who make the actual boards. However, most boards are reference design, which is simply copied from what NV cooked up for a finished product. So in a way, they can blame the manufacturing companies for boards, or blame TSMC for chips, doesn't make you many friends though... In the end, the board designs and chip designs are NV's, they need to own up to any problems.
