Default Systematic list of blank screens with 6629 -- tnt2 and mx 440

We've all been griping at NV for the 6629 drivers, esp. wrt to broken TNT2 and (some?) MX 440 cards. Those two seem to be consistent. Others seem to be intermittent. What I recommend is for everyone to post some real info, and not just "me too" -- we've already proven that the issue exists -- lets show NV that the community can help with fixing things too. I've got a TNT2 that shows up all the symptoms -- black screen with some fuzz in the top left, can see cursor, etc. I'm going to post three pieces of info, and I recommend that if you think you've got the problem, post the same.

PS -- I'm going to get an old MX 440 from a friend who is upgrading tonight, so I'm going to be able to add to this list. Please try your best to contribute. If you have a TNT2 or MX 440 that WORKS, please post up the details as well.
