edit: turns out my SSC card was bad (ie, could not reach 1458 shader even with 100% fan.)

What all programs do you have installed to control it?
Make sure all are set to auto Is this a new card, or did it work previously? Could be the drivers.

Vanilla Vista SP1 install, vanilla nVidia drivers from their website. I did download evga precision and riva tuner to confirm auto mode. It's pegged at 40% for low, medium and high.
