Default ti4400, 8rda+, mdk 9.1

sorry for bad english, but:
i have tried several versions of nvidia drivers and non will work anymore, when i first time installed them they worked nicely, but after one reboot they just didn't worked anymore
now when i'm trying to use the driver nvidia, i just have blank screen and hole system is crashing, even the ctrl+alt+del won't work!

the last lines in XF86 log are:
(EE) [GLX]: Failed to add GLX extension (NVIDIA XFree86 driver not found)

then it just won't work, if i'll try to load nvidia module by hand using modprobe nvidia command, sometimes it works, sometimes it NOT work... it says something about tainted module and blaa blaa...

the hardware is:
epox 8rda+
2200 axp
628 ddr
ti 4400(128mb)
mdk 9.1

i'm not going to put the confs and others to here, bicous they are correct bicous it once worked with this conf, i have also tired that agp 0 system, it said "not using" so it's correct?

please help me, wantto play to rtcw

at least post your /var/log/nvidia-installer.log and entire /var/log/XFree86.0.log. There cannot be too much information, well there can be, but more is helpful, in this case.

i said already that the conf is okay, the problem isn't there, its now nv, you're right in that, but chancing it doesn't make it better, worse, the hole ****ing muchine crashes... and thats the weirddesst that it once worked!!! then i rebooted my machine(can't remember what did i done before boot) and it just doesn't worked!
