Default VDPAU gets into a bad state

After watching a few videos (typically with XBMC) using VDPAU, something in the VDPAU stack gets into a bad state such that focusing on the window playing the video—whether XBMC or mplayer—causes the video and all overlays, including the mouse cursor, to stop. This, of course, means that I cannot interact with the on-screen controls, making it impossible to use XBMC. I can subsequently play videos using mplayer with Xv (with the attendant high CPU usage, tearing, and stuttering), but I need to reboot in order to reset the VDPAU state.

FWIW, I am using four heads on two cards (GTS 250) with Xinerama, so IIRC the VDPAU codepath I'm using is different from that used on single head or TwinView setups, which may be why I haven't found any other complaints about this problem.

Any ideas on how to further debug this?
