Default Quadro4 400NVS and more than 2 displays

I am having a problem with a NVIDIA Quadro4 400NVS controller getting more than 2 displays (all DFP) working with NVIDIA's linux drivers (1.0-6111 for Linux IA32) for XFree86 on a Red Hat Linux 9 system.

The Digital Flat Panels (DFP) are Samsung SyncMaster 191T+ (plus).
Videocard: NVidia Quadro4 400NVS
System: HP/Compaq single P4 workstation with hyperthreading
Linux distribution: Red Hat Linux 9 (for i386)
Linux kernel: 2.4.20-31.9.progeny.3smp
X11: XFree86 4.3.0-2.90.55
compiler: gcc 3.2.2

(II) NVIDIA(0): NVIDIA GPU detected as: Quadro NVS

I tried TwinView and UseInt10Module and both didn't help. Attached is the "best" I can get so far workingwhich is 2 DFP displays working with 3 actual DFP attacted.

How can I get the third display to work? I assume the fourth display woudl be a simple extension.

I updated the Video BIOS (to 04.17.20.38.30), Linux kernel version to 2.4.20-31.9.progeny.5smp, and System BIOS (HP d530 BIOS from 1.11 to 2 .43 A) with no effect.

It works after I discovered that "Screen" means different things in different sections.
